Duties and Responsibilities
- Conducts discharge assessment in order to evaluate patient’s current and future concerns. Facilitates and coordinates post-hospital care.
- Coordinates discharge planning of patients that are being discharged from acute care or skilled nursing facilities.
- Provides authorization for Home Health (PT/OT/Skilled Nursing Visits), home infusion, hospice care, Durable Medical Equipment (DME), medical supplies, formula, and self-injectables as ordered by the attending physician and directed by the Inpatient Nurses; and if meets criteria for medical necessity. Assigns appropriate payor code accordingly by checking the DOFR (Division of Financial Responsibility).
- Coordinates and provides authorization for outpatient dialysis post discharge (for new dialysis patients), outpatient PCP and specialty follow up needs.
- Verifies Discharge Disposition (Home, Home Health, Hospice, SNF, etc.).
